HOW TO REGISTER BY FAX?

Step 3: Please note that you will need to bring your check with you onsite. Any checks received after 10/25/12 are unable to be processed before the Symposium.
Once your registration has been received and processed, a confirmation email will be sent to you with receipt of payment.
*All payments must be received by the early bird rate deadline, not postmarked, to receive all early rates. Any registration with outstanding payments by the deadline will be moved into the next registration rate category.*
Cancellations must be received in writing to firstname.lastname@example.org. A processing fee of 50% will be charged for cancellations received by September 30, 2012. No refunds will be issued for cancellations received after September 30, 2012. No refunds will be issued for no-shows. There is no charge for substitutions. Eligibility requirements for member rates will apply. Badge-sharing is not allowed.
