Purpose/Goal:

Transplant outcomes are pertinent for a program’s success. Outcomes depend on many controllable and often uncontrollable factors. Patient selection is a key process to minimize poor post-transplant outcomes through the selection of appropriate patients for a complex regimen. A patient focused multidisciplinary team can create excellent outcomes through candidate selection, patient and caregiver education programs and transitions of care to the post transplant setting. This topic will focus and both pre and post-transplant strategies to maximize patient adherence.

Target Audience:

Members of the donation and transplantation community serving diverse populations to include administrators, coordinators, physicians, nurses, surgeons, managers, quality improvement specialists, social workers, and other donation and transplantation center professionals and their colleagues.

Learning Objectives:

By the end of the webinar the participant will be able to:

  • Describe the design of a pre-transplant multidisciplinary team.
  • Summarize the concepts of patient health literacy and how it applies to transplantation.
  • Evaluate current post-transplant multidisciplinary care practices related to patient adherence and outcomes.
  • Identify strategies to maximize post-transplant patient adherence.
