Learn about real successes using a new, comprehensive community care model of diagnosing and addressing poverty. Join Marcella Wilson, Ph.D., national leader of Matrix Transition to Success and author of "Diagnosis Poverty" and two directors from model sites piloting this new care model as they discuss the "diagnosis" model and how to implement it successfully. Funded through The Kellogg Foundation, the pilot sites show significant success in treating and alleviating poverty. Hear how two organizations, United Way of the Mid-South in Memphis, TN and Child and Family Services located in Hawaii, have implemented this model to diagnose and address poverty in their respective communities. Both organizations will share their successes and provide independent data sets to validate the outcomes presented.
